Anterior uveitis

one of the causes of acute Red eye
Etiology HLA-B27 association e.g. Crohn's disease, Ulcerative colitis; also Multiple sclerosis
Epidemiology
Clinical presentation redness, photophobia, tearing, pain (anterior) - onset within hours
O/E: synechiae (= misshapen pupil), unreactive pupil, hypopyon

- Shining a light into the ‘good’ eye causes pain in the other. Pain ↑ as eyes converge and pupils react to accommodation (Talbot’s test)
Pathogenesis
Diagnostic investigations Exclude corneal abrasion/ulcer, check IOP.
Slit lamp: aqueous flare in ant chamber
Check HLA-B27 if recurrent.
Management Refer ophthal
Treatment may include steroids, cyclopentolate/atropine (paralysis of ciliary body to prevent adhesions between iris and lens)
