Transfusion reactions
classify or die
| Acute | Delayed |
|---|---|
| - immune-mediated - acute haemolytic transfusion reaction - febrile non-haemolytic transfusion reaction - TRALI - allergic reaction - non-immune - TACO |
- immune - delayed haemolytic - GvHD - non-immune - iron overload |
febrile non-haemolytic transfusion reaction
| Fever without systemic upset is more reassuring | |
|---|---|
| Etiology | Immune response to WBCs in donor plasma |
| Clinical presentation | - Mild – temperature ≥38°C or 1-2°C above pre-transfusion baseline - Moderate – temperature ≥39°C or ≥2°C above pre-transfusion baseline In an otherwise well patient |
| Management | PO paracetamol is acceptable |
TACO
treat as regular old pulmonary oedema. stop the transfusion, naturally.
